Abstract

This chapter presents a wonderful and systematic approach to elicit various symptoms and signs of commonly encountered clinical problems. It gives you detailed questionnaire regarding how to inquire about various signs and symptoms of the diseases. Painful efforts have been done for the readers so that they can beautifully and easily perform their tasks of history taking. Sincere and special efforts have been undertaken regarding some common signs and symptoms (but tough for the students and clinicians to elicit) like Pain, Vomiting, Diarrhea, Dyspepsia, Hematochezia, Halitosis, Hematemesis, Dyspnea, Syncope, Pyrexia, Arrythmia, Headache, Polyuria, Gait and many more. Sometimes the patients (though willing) find themselves helpless in giving details (both symptoms as well as their etiology) of various clinical problems viz. Apraxia, Aphasia, Reflexes, Dementia and confusional states, Emotional states, Spontaneous fall, Visual defects, Sensory systems, Ataxia, Cerebellar signs, Neurological problems like urine and stool incontinence, Wheeze, Orthopnea, Malena and so on. It is the clinician who has to find answers to all these problems/illnesses. And, for this, the first and the foremost step is- “PROPER AND EXPERT HISTORY TAKING”. This chapter will help all the readers to get answers to their yet unanswered questions …!!!
