Abstract

A case history of prolene stitch protruding through the bladder, after laparoscopic Burch surgery is described. The stitch was cut cystoscopically. The author urges readers to clearly differentiate genuine stress incontinence and avoid cases with detrusor instability. Some amount of incontinence should be expected after surgery, and thus, overcorrection avoided. Technical failure of Burch could result from misplacement of sutures, inadequate fixation of parvaginal tissue to Cooper’s ligament, or sutures pulling out of tissue. Burch is recommended only for patients with genuine stress incontinence with prophylactic obliteration of pouch of Douglas who has intact urethral sphincters. Trocar site herniation is described as a complication of extra-peritoneal Burch, while Bladder injury, Bleeding, and urethral injuries are cited as complications of Intraperitoneal Burch. Genital prolapse is cited as a late postoperative complication of Burch surgery. Methods of avoiding these complications by using correct surgical techniques are described.
