Hypertensive Disorders in Pregnancy

by Milind R Shah

Hypertensive Disorders in Pregnancy is second most cause of maternal death after haemorrhage in many parts of world. Approximately 1,00,000 women die worldwide per annum because of eclampsia. It is said that pre-eclampsia and eclampsia contribute to death of a woman every 3 minutes worldwide. This extremely good book concerns hypertensive disorders in pregnancy: a very complex situation that an obstetrician has to deal with in his day-to-day practice. Essential hypertension and renal hypertension, though more sinister but fortunately rarer than pregnancy-induced hyertension (PIH), are rightly dealt with at the beginning. Aetiopathology of PIH has been controversial throughout the last more than hundred years. It is now getting clearer yet controversies persist as explained in the relevant chapters. Immunology and genetic aspects are nicely depicted. From a clinician’s point of view, the most important areas are evaluation of fetal condition and assessment of damage to maternal organs. These are methodically discussed and the utility of sonography, colour Doppler and electronic fetal monitoring is well worked out. Difficult decisions regarding timing, mode, and conduct of delivery are very crucial ones and they are adequately covered. Eclampsia is preventable with good antenatal care and is becoming infrequent. But it is still encountered, especially in rural practice and its management is well described in detail; while prophylaxis, preventive therapy and long-term prognosis are properly handled in a thought- provoking manner. On the whole, this excellent book very ably covers each and every aspect of PIH in a practical manner. This book is useful for every obstetrician, clinicians, academicians and postgraduate students.
