Abstract

The preoperative consultation for a patient who has requested refractive surgery is not entirely different from a general consultation; specific examinations are performed in addition to the general ones. Our idea of team is fundamental: a series of professionals who work together toward the same aim. Following the preoperative examinations, the patient comes in contact with the patient care staff. The capacity to establish relationships should be considered a fundamental element in forming a bond and creating situations of reciprocal understanding. Obtaining the patient’s written consent authorizing surgery should be part of the preoperative phase during which the patient reviews the different phases of the pending surgery. In our institute the patient enters the operating room after the instillation of anesthetic drops in both eyes; he is requested to wear a headgear and top-boots. After the suction, the surgeon through the microkeratome, creates the flap, then the suction is turned off and the microkeratome is lifted, the flap is lifted and positioned nasally, the ablative phase can start; finally the flap is repositioned. After the first eye we perform Lasik on the other eye. The patient now can leave the operating room for postoperative care. One of the most critical factor that can affect the outcome after surgery is the nomogram that the surgeon uses: it is a unique group of detailed settings that is programmed into the laser for each patient. The postoperative phase is probably the most delicate phase for the patient because it reveals his/her hidden expectations.
