Abstract

The population of elderly people is going to increase in future. The number of elderly people in India is projected to be 179 million by the year 2031 and 301 million by the year 2051. This rising trend is as a result of better medical facilities and improved socio-economic status. Though some of these elderly individuals may remain physiologically young, a large percentage of this ageing population may have chronic diseases like diabetes mellitus, hypertension, coronary artery disease, chronic respiratory diseases, etc. The physicians, in general and the anaesthesiologists, in particular, should be aware of these facts and should be geared up to look after this population who may turn up for surgery either by open or by the laparoscopic approach. With the addition of every decade, the anaesthetic challenges increase in magnitude as these elderly patients have diminished physiological reserves and increased underlying comorbidity. With the increasing success of the laparoscopic procedures, surgeons of different specialities are adopting the laparoscopic approach to various conventional surgeries even in the geriatric age group. Apart from general and gastrointestinal surgeons, these procedures are also being performed with great enthusiasm by the gynaecological, urological and vascular surgeons. The anaesthesiologists face additional challenges when laparoscopic procedures are performed in the geriatric age group. The rapidly expanding number of newly developed laparoscopic procedures in different surgical specialities, especially in elderly patients, requires anaesthesiologists to familiarize themselves not only with the physiology and the potential complications of the pneumoperitoneum but also with the unique requirements of different surgical procedures. In near future, these laparoscopic procedures will move on to even more complicated and reconstructive surgery, requiring exposure to pneumoperitoneum for a prolonged period of time. With the increase in the population of elderly people and popularity of the laparoscopic procedures, more and more elderly patients will be posted for laparoscopic surgeries. The anaesthesia should be conducted meticulously and the role of proper monitoring cannot be over emphasized.
