Abstract

The turn of century has seen new strides in assisted reproductive technology and it has lead to development in other fields like molecular biology and reproductive endocrinology. Ovarian reserve should be tested before subjecting the patient to ovulation induction. Three most important parameters indicating poor ovarian reserve are advanced age, high basal FSH and low antral follicles. GnRH antagonist (ganirelix, cetrorelix) have recently been made available and they are not associated with the initial flare effect unlike the agonists. New ooplasm and nucleus transfer techniques have been developed to craft young oocytes from older ones without disturbing the nuclear genome of the older woman. In vitro maturation of oocytes is another technique which circumvents the need of inconvenient and costly stimulation of ovaries. Assisted hatching is indicated in older women with high basal FSH and thick zona. Pre-implantation genetics use has now increased in couples with repeated IVF failures and miscarriages to exclude embryos with aneuploidy and thus holding a promising outcome for them.
