This book shows how medical imaging is being increasingly used to define regional anatomy, function and chemistry, that latter being based chiefly on positron emission tomography. They make possible a molecular theory of disease, a molecular approach to diagnosis and, in many cases, treatment. Characterization of disease has moved to a more basic level from the cellular to the molecular level. This book describes quantitative imaging of the spatial and temporal distribution of radio labeled molecules within the living body by means of scanning instruments outside the body. Molecular processes in different parts of the body then define the patient’s problems. Long after an important happening has occurred, most people do not say “I was for it or I was against it,” but rather “I didn’t know it was happening.” What is described in this comprehensive book is a whole new approach to the definition of diseases, including the diagnosis of “presymptomatic disease” in those persons identified as being at high risk of subsequent development of disease. This is best illustrated in the chapter “Insulin Resistance Syndrome/Metabolic Syndrome”. For centuries, it has been recognized that many diseases come from forces outside the body, that is, from microbes, trauma, violence, and deficient diets. Now it is recognized that disease can come from innate processed within the body, from abnormalities in the molecular processes inherited from our parents and affected by external environmental forces. This book shows how we can assess the molecular processes involved in energy production and communication in different parts of the living human body in health and disease. We can address a patient’s problems in terms of regional function and biochemical processes, and try to answer the four basic questions of the practice of medicine: What is wrong? What is going to happen? What can be done about it? Is the treatment effective?
