Abstract

This chapter discusses various aspects of gynecologic cancer surgery, providing an appreciation and understanding of the surgical principles of the subspecialty of gynecologic oncology. Surgical management of cancer cervix, surgical treatment of malignant ovarian neoplasms, surgical management of carcinoma vulva, pelvic exenteration, and laparoscopic surgery in oncology, are some topics discussed in detail. Pelvic radiotherapy is found to effectively reduce the relapse risk when surgical margins are close or positive. Cases of carcinoma of the corpus should be grouped according to the degree of differentiation of the adenocarcinoma. It is recommended that except patients of stage Ia G1/G2 and stage Ib G1/G2 disease, all other patients should be offered postoperative radiotherapy as an adjuvant therapy. In stage III disease of malignant ovarian neoplasms, every effort should be made short of major bowel surgery to remove the bulk of the tumor, including the large omental cakes. The proximity of relatively radiosensitive normal tissues, such as the bladder and the rectum, provides a real challenge to the radiotherapist, especially in dealing with tumors of the lower third of the vagina.
