Abstract

Ovarian cancer usually spreads by exfoliation of malignant cells into the peritoneal cavity which gets implanted to various sites via the circulation of peritoneal fluid. Lymphatic spread can occur to the paraaortic, pelvic and rarely to the inguinal nodes. Spread through the retroperitoneal lymph nodes lead to dissemination of the disease above the diaphragm especially to supraclavicular lymph nodes. Pelvic lymphadenectomy involves removal of peri-vascular lymphatic tissue from common iliac, external iliac, and internal iliac vessels Internal iliac group of lymph nodes include obturator nodes also. The retro peritoneum is entered by cutting the round ligament and incising the peritoneum lateral to the ovarian vessels up to pelvic brim.
