Abstract

Prognostic factors aid in selecting the optimum treatment for ovarian cancer. Even though age, stage, performance status, histopathological subtype and the volume of residual disease following laparotomy are the important factors. Capsular invasion, surface excrescences, tumor rupture and positive peritoneal cytology seem to provide additional information. Good performance status is a good prognostic factor while mucinous and endometroid subtypes fare better when compared stage by stage. Serum CA125 levels and kinetics correlate with tumor load and suggest treatment response. Salvage surgery is an option for some relepsers while stage, grade and extent of residual disease are significant predictors of recurrence. Some subjects attain meaningful improvement in survival following negative second look. The role of High dose chemotherapy followed by PBSCT has been investigated as yet another technique for consolidating the gains obtained following salvage and is an investigative option. A plethora of novel prognostic factors are being investigated to aid proper selection of treatment for epithelial ovarian cancer.
