Anatomy of Thoracoabdominal diaphragm is important for a gynecology oncology surgeon since diaphragmatic peritoneum is a preferred site of metastasis from ovarian cancer and hence diaphragmatic stripping is a part of cytoreductive surgery if involved. The process of ovarian cancer spread is analogous to scattering sand across a surface. The peritoneal current is said to be clockwise, the cancer cells are carried to nook and corner of the abdomen. Cancer cells are also easily absorbed at the stomata in the diaphragm. Diaphragm originates as 3 parts, insertion is achieved by the convergence of all these on the margins of large aponeurotic central tendon. Diaphragm contains hiatuses through which a number of important structures pass on to either side.
