This discussion highlights the grey-zone of cytomorphologic spectrum which overlaps with high-grade squamous intraepithelial lesion (HSIL) in cervical cytology. Some cases in this spectrum may have to be interpreted as ‘atypical squamous cells, cannot exclude HSIL’ (ASC-H) or as ‘low-grade squamous intraepithelial lesion (LSIL) along with some features of ASC-H’ (LSIL-H). A brief description highlights future trends in this area in particular and cervical cytology in general. Extensively cited bibliography is additional resource for readers to explore in depth. Liquid Based Cytology is popularized in USA and in some developed countries and is said to give increased sensitivity and afford material for HPV Testing. However, well performed conventional Cytology is still the effective and economical tool for cancer detection. It has shown drastic reduction in incidence and mortality rates of cervical cancer when mass screening programmes are successfully conducted.
