Abstract

This chapter discusses microbiology and medical management of bacterial sinusitis, where the current principles of management of bacterial sinusitis include the accurate establishment of the diagnosis using clinical information, prescribing, and treating the patient with an antibiotic chosen for predicted probability of success and that reduces the potential of antimicrobial resistance. The upper respiratory tract including the nasopharynx serves as the reservoir for pathogenic bacteria that can cause respiratory infections including sinusitis. Viral infection is the most common predisposing factor for upper respiratory tract infections, including sinusitis. When chronicity develops, the aerobic and facultative species are gradually replaced by anaerobes. Nosocomial sinusitis often develops in patients who require extended periods of intensive care involving prolonged endotracheal or nasogastric intubation.
