Abstract

This chapter discusses other nonsquamous tumors, where adenoid cystic carcinoma accounts for approximately 10 percent of all non-squamous cell carcinomas in the head and neck and for 15 percent of all neoplasms of the salivary glands. Adenoid cystic carcinoma arising from the minor salivary glands are often advanced at the time of diagnosis, and complete excision is limited by their large size, and proximity of the tumor to important neural and vascular structures as well as essential functioning organ. The patients who had a solid histologic pattern of adenoid cystic carcinoma appeared to have an overall worse prognosis in terms of distant metastases and long-term survival. Perineural spread can occur in both axial and circumferential pattern along the involved nerve, and further spread can occur both in an antegrade and in retrograde fashion. Metastases to the regional lymphatics are more common amongst tumors from the parotid glands than from the minor salivary glands.
