Abstract

This chapter discusses margins in head and neck cancers, where fringe of tissue surrounding the tumor should be adequate and is limited by the narrow anatomical relationships in the region of the head and neck. The pattern of invasion of SCC of the head and neck into the soft tissue can be divided into four groups, where first group is characterized by a broad tumorous front, the second group is characterized by the occurrence of plump tumorous fingers at the tumor front while the third subsumes infiltration via ramified tumorous fingers and islands. The fourthr group is defined by the preponderance of a diffuse disseminated pattern of infiltration. Intraoperative use of frozen sections may help in achieving more margins free of tumor because patients may benefit from immediate margin revision. Bony margins are much more difficult to assess clinically and pathologically as compared to soft tissue margins.
