Abstract

Ankle TB is rare, the incidence is less than 5%. Early stage of disease is best treated by anti-TB drugs, intermittent active and assisted exercise. The commoner sites of involvement of foot bones are calcaneus, subtalar and mid-tarsal joints, first metatarsal and navicular bones. In patients not treated in early stage the tuberculous process spreads rapidly too many parts of foot bones because of intercommunicating synovial channels. Multidrug therapy and a suitable plaster cast or orthosis would heal the disease in the best functional position. One should aim at neutral plantigrade ankle with 5 degree of external rational at ankle joint and 5 degree of valgus of heel. Operative steps of ankle synovectomy, ankle arthrodesis, and debridement of foot bones are described, with relevant surgical anatomy.
