Abstract

Visual rehabilitation of pediatric corneal blinds is a major challenge to corneal transplant surgeons. Penetrating keratoplasty is the only way to restore vision and prevent irreversible blindness because of amblyopia in children. Indications of penetrating keratoplasty may be grouped into congenital corneal opacities and acquired corneal opacities. Corneal edema due to endothelial cell decomposition in patients with buphthalmos is successfully treated by penetrating keratoplasty. Recent studies show that penetrating keratoplasty in children should be performed at the earliest to prevent irreversible amblyopia. Detailed examination of infants and neonates with congenital corneal opacities is essential to plan treatment. Investigations, pediatric keratoplasty constraints, preparation before corneal transplant surgery, donor tissue, and surgery are discussed. Size of the graft, alternatives to penetrating keratoplasty, high risk penetrating keratoplasty, perioperative care, complications, and outcome are described.
