The “Big Bubble” Technique allows safe and consistent exposure of Descemet’s membrane. The visual results achieved are comparable to those of penetrating keratoplasty. Long-term complications are greatly minimized. Corneal surgeons should be encouraged to use this technique in cases where the endothelium is healthy. Planned exposure of Descemet’s membrane by the Big Bubble Technique. This provides safe, speedy and consistent exposure of the smooth and shining surface of Descemet’s membrane. On the donor side Descemet’s membrane is peeled off by non toothed forceps resulting in another uniform and smooth surface. The apposition of the donor button to the bare Descemet’s membrane provides an interface of high quality. It is similar to the natural pre Descemet’s membrane plane of a normal cornea. Thus, it does not interfere with the visual acuity. The procedure is standardized because the interface achieved is exactly the same in every case. Once, the Big Bubble is achieved, it is relatively safe and straight forward to bare Descemet’s membrane even by an average surgeon.
