Abstract

This chapter describes the incidence, etiopathology and the clinical diagnosis of each of the two conditions. The problems of multiple pregnancy and polyhydramnios pose a unique challenge in the practice of obstetrics. The practical problems encountered in the management strategies have also been outlined. The successful outcome of these high-risk pregnancies requires that the clinician coordinate with an experienced group including anesthetists and neonatologists. Despite difficulties, with modem treatment modalities the maternal and perinatal outcomes are good in most cases. Many obstetrical problems arise in multiple pregnancy like preterm labor, intrauterine growth restriction, hydramnios, maternal conditions like anemia, diabetes, hypertension increases thereby adding more problems. Hydramnios or polyhydramnios is a pathological condition characterized by an excessive accumulation of amniotic fluid, usually more than 2000 ml. The conditions associated with polyhydramnios include maternal (15%), fetal (18%) or placental (1%) causes. In the vast majority of the cases (65%) the etiology is idiopathic. Fetal anomalies account for 12.7% of the cases of polyhydramnios. Abnormalities of the central nervous system include anencephaly, encephalocele, spina bifida are some of the common defects giving rise to hydramnios, diabetes to be ruled out.
