Abstract

This chapter briefly discuss on malpositions and malpresentations. In the developed countries, one of the big advancement of modern medicine in the 20th century has been the ability of controlling treatable and preventable causes of perinatal and maternal mortality. Unfortunately, in the developing world like ours, mothers and their offspring are still dying of easily preventable but rapidly fatal pregnancy-related causes; considered to be disease of the 19th century in the developed world. The commonest presentation of the fetus in late pregnancy is cephalic. This is not because of the shape of the pelvis or the fetal head, but because of the pyriform-shaped cavity of the uterus (more so in the primigravidae), which allows for more room for lower limb activity in the fundal region. But sometimes, malpositions like occipitoposterior and malpresentations like breech, face, brow and transverse lie occur. Malpresentations like brow and shoulder have no mechanism of labor and they need to be identified and cesarean section needs to be done as it will cause obstructed labor, rupture uterus and its complications.
