Abstract

Since the first two aspects of puerperal problems (secondary postpartum hemorrhage and puerperal sepsis) this communication will briefly address the other encountered problems, both common and uncommon, during the puerperium. The puerperium is indeed a time of great importance for both the mother and her baby, and yet it is an aspect of maternity care that has received relatively far less attention than pregnancy and delivery. The problems of the puerperium are diverse and can be conveniently discussed under the following broad headings: 1. Secondary postpartum hemorrhage, 2. Puerperal sepsis, 3. Breast, 4. Venous system, 5. Urinary tract, 6. Chest, 7. Mental health, and 8. Genital tract. Puerperal sepsis and secondary hemorrhages are the common problems in the puerperium. Infection of the breast (synonymous with infectious mastitis) occurs almost exclusively in lactating women. Any fever of > 38ºC on any 2 of the first 10 days postpartum, except the first day. The postpartum period is the most common period during pregnancy for a thromboembolism. The most common place for a clot/embolus to get deposited from the pelvic veins is in the pulmonary circulation. About 2 to 4 percent of women develop urinary tract infection postpartum.
