Abstract

This chapter discusses evolution of glaucoma surgery. Glaucoma is defined as optic head neuropathy with characteristic morphological and functional changes or defects for which the major risk factor being increased intraocular pressure. Classification of glaucoma surgeries include operations to relieve intraocular block of aqueous humor, operation to form paths for intraocular drainage of aqueous humor, operations to improve extraocular drainage of aqueous humor, filtering operation without sclera flap, filtering operations with scleral flap, operations to reduce production of aqueous humor, and seton surgeries. Ancient glaucoma surgeries are discussed. Practiced glaucoma surgery includes laser glaucoma surgery, goniotomy, trabeculotomy, goniosynechiolysis, trabeculectomy, shunts, and non-penetrating glaucoma surgery (NPGS). The types of NPGS are sinusotomy, deep sclerectomy, viscocanalostomy, and canaloplasty. Deep sclerectomy defined as a procedure that surgically opens Schlemm’s canal, exposes Descemet’s membrane, and bypasses the site of highest resistance to outflow in the juxtacanalicular trabecular meshwork. Viscocanalostomy involves production of superficial and deep scleral flaps, excision of the deep scleral flap to create a scleral reservoir, and unroofing of Schlemm’s canal. The future of glaucoma surgery and fibrin glue in glaucoma surgery is discussed.
