Abstract

In this technique, cataract Surgery is performed through 3.2mm. clear corneal or 3.5 mm. scleral tunnel incisions. Post operative astigmatism is similar to that obtained with phaco but with less learning curve. After hydrodissection, nucleus is fragmented into multiple tiny pieces of 2 × 2 mm with the help of spatula and nucleotome. Spatula is kept beneath the nucleus and nucleotome above the nucleus. Pressure is created by slowly pressing nucleotome against the spatula to break the nucleus into 4 pieces. Pieces are removed with sandwich technique. Right and left manipulation is used to displace remaining fragments to center for further fragmentation and removal. This method enables surgery in both soft and hard nuclei. Nucleotome is racquet shaped instrument, 8 mm long and 2 mm wide divided along its short axis by 3 transverse bars. Spatula is 8 mm long and 2 mm wide.
