Three special instruments required for this technique are anterior chamber maintainer (ACM) aspiration cannula and nucleus removal tube (Am porn tube). Two paracentesis at 6 and 12 O’ clock is performed, 6 O’clock for ACM and 12 O’ clock for viscoelastic injection. A corneal incision of 3.5 mm is made temporally. After hydrodissection, nucleus is cracked into 4 pieces in the capsular bag with rhexis forceps and Sinskey hook. Pieces are removed via nucleus removal tube by creating passive aspiration force which is inserted through corneal wound. Main advantages are less wound trauma as pieces pass through tube, cheap and no heat production. Disadvantages are only selected cases (NS grade 1 + to 3+) can be done, increased surgery time, requires learning and needs special instruments.
