SlCS is difficult in cases of small pupil. There are many techniques for dilating small pupil either individually or in combination, e.g. mydriatic drugs, viscomydriasis, synechiolysis, membranectomy, laser photomydriasis, stretch pupilloplasty, iris hooks, ring dilators and iris surgery. In such cases capsulorhexis can be done by pull and push method, then hydrodissection should be done with minimal amount of BSS. Nucleus prolapse is done by bimanual technique. Nucleus can be removed by any method (e.g. irrigating vectis, sandwich, etc.). For IOL implantation, 2 types of viscoelastics, i.e. cohesive and adhesive are used. More inflammation is expected post operatively which is treated by steroids, cycloplegics and antiglaucoma medication.
