Cortical aspiration is very important step in cataract surgery. For this, coaxial retro illumination is a must. It can be done using either automated system or manual irrigation aspiration devices. Aspiration always proceeds from periphery to the center of pupil. While aspirating, the aspiration port must be visible, i.e. face upwards. Subincisional cortex is most difficult to aspirate which can be aspirated by J or U shaped cannula or by side-port made 70° to 90° away from primary incision. Complete removal ensures quick visual recovery, decreases chances of postoperative uveitis, PCO formation and IOL decentration.
