Abstract

A patient in whom the crystalline lens or IOL is dislocated posteriorly into the vitreous may present with vitreous hemorrhage, endophthalmitis or aphakic glaucoma, “Double IOL Syndrome” characterized by a dislocated posterior chamber IOL and an anterior chamber IOL, retinal detachment and severe inflammatory reaction. An ultrasonography, detail indirect ophthalmoscopy and slit lamp examination along with recording of the intraocular pressure must be undertaken. Identification of the problem and early referral to a vitreoretinal surgeon must be done. Simple pars plana vitrectomy usually suffices or when combined with intravitreal phaco emulsification or intravitreal phacofragmentation or modified 25G needle approach or MVR blade delivery of the nucleus.
