Abstract

Cataract and glaucoma are common conditions and may present in same patient. Combined cataract surgery and trabeculectomy have shown to be an effective procedure in these patients. It is often difficult to evaluate the optic nerve head and visual field in presence of cataract where previous records may be helpful. Pre operative evaluation of subluxated lens, atonic pupil, pseudoexfoliation syndrome is also important. Fornix based conjunctival flap is preferred because of better exposure and posterior development of bleb. Scleral dissection is kept at half the thickness and the anterior entry wound should extend little anterior to cornea. Size of capsulorhexis is limited by small pupil so multiple partial sphincterotomies or stretching of pupil with Sinskey hook may be required. Small incision techniques offer distinct advantage over it with safe removal of the nucleus. ACIOL should be avoided in these cases. Use of mitomycin C may effectively limit fibrosis, scarring and bleb failure.
