Abstract

Posterior capsule opacification is a major complication of extracapsular cataract surgery. Most secondary cataracts are caused by proliferation of equatorial epithelial cells (E-Cells) and anterior epithelial cells (A-Cells). A similar clinical picture may be induced by localized endophthalmitis in which case if capsulotomy is done, severe inflammation may be precipitated. The degree of opacification is assessed by distant direct ophthalmoscopy, slit lamp biomicroscopy and decrease in best corrected visual acuity after surgery. Prevention of posterior capsule opacification can be done by cortical clean up, by hydrodissection and by factors depending on position of IOL, its biocompatibility and barrier effect. There are also pharmacological and immunological inhibiters for PCO. Management of PCO is mainly by Nd: YAG laser capsulotomy.
