Abstract

Sound surgical technique is based on accurate anatomic knowledge. During laparoscopy, pelvic anatomy can appear significantly different due to the effects of pneumoperitoneum, patient positioning and tissue manipulation to facilitate traction and countertraction during surgery. Additionally, there are inherent limitations of laparoscopy related to a fixed visual axis, loss of depth of field and magnification. The development of laparoscopes with different angles of view may facilitate visualization of certain structures, but may also make orientation more challenging. Since a three-dimensional field is projected to a monitor as a two-dimensional image, it is imperative for the endoscopic surgeon to understand that the anatomical structures appearing superior on the monitor are actually anterior and those inferior are posterior. In this chapter, we will describe some important anatomic relations that are critical during laparoscopic procedures.
