Abstract

In 1994, Luigi Fedele have described a laparoscopic version of the Vecchietti’s method, which has proven safe and effective. Other Authors have proposed different laparoscopic modifications of the Vecchietti’s operation but with small series. Results: All patients had a safe surgery. No intraoperative or postoperative complication were encountered. By day 8 a good 6 cm length of vagina achieved. Neovagina gets covered with stratified squamous epithelium almost 80% after about 3 months of surgery. Complete squamous epithelium growth is noted one year after surgery. Successful lubrication and coital function achieved from 3 months after surgery. Laparoscopic creation of neovagina appears to be a safe, simple, effective method of creation of vagina. The simplicity comes from the fact that there is no dissection involved in the rectovesical space and secondly there is no need of skin grafting. Safety is rendered by total laparoscopic approach as no step is blind. We can appreciate the limits of bladder and rectum at every step, so chances of inadvertent injury is very low. Efficacy has been proved by thorough follow-1up which shows that laparoscopic creation of neovagina gives anatomically and functionally gratifying results.
