Abstract

Various modalities exist, but sonography and that too endovaginal sonography has brought about a sea change in the preoperative assessment of gynecological patients. It is almost like “Eyes added to one’s fingers”. Endovaginal sonography offers the unique advantage of knowing much more about any mass, cyst or endometrial pathology. Adding color gives a much superior optimization of diagnosis of pelvic masses, especially adnexal masses. Since the role of laparoscopy in malignant conditions is still not fully established, all endeavors should be done in preoperative phase to establish that the mass is benign. The biggest advantage comes from an already existing doctor-patient relationship, so the patient is much more relaxed, gives in more details while doing sonography. Thus, endovaginal sonography is the single most important preoperative diagnostic modality and its accuracy and correlation improves, as the clinician carries out the scan in the office. Intra- and interobserver differences can be eliminated and no description by someone else can match upto what the clinician sees on the ultrasound monitor. As the Chinese proverb goes, one picture is equal to 10,000 words. The image is seen by the clinicians themselves, conveys a lot more about the understanding of the disease. We have been doing sonography since 1985 and endovaginal sonography since 1990 and over these years, found it the single most dependable tool without which we never take up patients for endoscopic surgery. Endometrial morphology is so well-depicted by endovaginal sonography that it becomes the most logical investigation.
