Abstract

The three most common malignant tumors of the eyelid include sebaceous carcinoma, squamous cell carcinoma and basal cell carcinoma, in order of frequency of occurrence. In the West, basal cell carcinoma is the commonest eyelid malignancy, followed by squamous cell carcinoma and sebaceous carcinoma. Infections of the orbit and periorbital tissues are an important subset of orbital inflammatory disease because they are potentially life threatening and demand prompt and specific management. Modes of presentation and treatment options differ according to the inciting factor, which can be bacteria, fungi or parasites. The differential diagnosis included rhabdomyosarcoma, fibromatosis, and fibrous histiocytoma. Electron microscopy confirmed the fibroblastic nature of the tumor cells in three cases. Follow-up ranged from one to 32 years (median, seven years). Two of five tumors recurred locally but none metastasized. Apparently surgical excision is the treatment of choice and the tumor has a favorable prognosis. Giant cell angiofibroma resembles solitary fibrous tumor and giant cell fibroblastoma and should be considered in the differential diagnosis of spindle-cell tumors in the eyelid, orbit, and conjunctiva. Orbit and eyelid hemangiomas need to be carefully evaluated by an ophthalmologist. Extraconal and intraconal hemangiomas are more frequently associated with ocular involvement.
