Abstract

The surgical oncology patient often requires vascular access for the ongoing administration of chemotherapy and parenteral nutrition. Many of the agents used are vesicants and can result in local tissue damage if extravasation were to occur, as well as phlebitis and venous thrombosis if infused in smaller veins. Administration therefore requires central venous access in order to avoid such complications and provide large bore access. In addition, many chemotherapeutic regimens require periodic treatment and as such, long-term central venous access is often necessary. The purpose of this chapter is to review the current indications and techniques of vascular access for the surgical oncology patient. Vascular access is essential and at times challenging in the surgical oncology patient to maintain chemotherapeutic and nutritional therapy. Although jugular access is preferred, other access sites can be successfully used. Sterile technique, line care and adjunctive endovascular interventions are key elements of catheter maintenance and contribute to prolonged access longevity.
