Abstract

Carcinoma vulva is the fourth most common female genital tract malignancy. There has been an increase in the incidence in the past few decades. This can be attributed to the increased life-expectancy of the female population and the increased incidence of HPV infection. DNA of HPV has been identified from pre-invasive as well as invasive lesions of the vulva. Chronic immunosuppression, systemic hypertension, diabetes mellitus and obesity are seen associated with vulvar cancer but no definite etiological link has been proved yet. It is reported that 13% of cases are associated with cancer in other sites like cervix, vagina and anal canal. This may be due to the common etiological factor, HPV infection. With better understanding of the lymphatic drainage of vulva, the surgical approach to vulvar cancer has drastically changed. The present surgical approach is more conservative. The advances in radiotherapy and chemotherapy have also contributed to the less radical surgical approach. SCC is the commonest form of vulvar carcinoma, amounting to 87%. The second most common being malignant melanoma. The less common types of vulvar malignancies are bartholin’s gland adenocarcinoma, sarcoma, basal cell carcinoma and invasive paget’s disease.
