Abstract

Preterm labor, PPROM, post term pregnancy, intrauterine fetal death and Rh isoimmunization are few important late term pregnancy complications. Preterm labor is one of the major causes of perinatal morbidity and mortality. Definition, incidence, risk factors, pathogenesis, prediction, prevention and management of preterm labor and preterm neonate are important aspects of preterm labor, each of which is explained in the detail. Diagnostic and management guidelines of Premature rupture of membrane (PPROM) according to gestational age is summarized in the chapter with help of flow charts. Postterm pregnancy poses diagnostic and management dilemma which is cleared in the chapter by giving clear message on diagnostic criteria and selection of patient for expectant observation/induction of labor or cesarean delivery. Intrauterine fetal death is another challenging situation in obstetrics which should be managed with compassion and patience. Etiology, diagnosis, management options, complications and postpartum examination of fetus is fully explained. Rh isoimmunization can cause wide range of fetal complication like neonatal jaundice, erythroblastosis fetalis and hydrops fetalis. Etiopathogenesis of Rh incompatibility and fetal and neonatal complications along with conventional and newer diagnostic and management protocol are well illustrated. Recommendations for Anti D vaccinations, indications and procedure of intrauterine transfusion are summarized clearly.
