Abstract

The congenital glaucoma is an abnormality in the development of the structures of the anterior chamber of the eye (including the angle), that leads to an obstruction in the flow of the aqueous humor, that will lead to an abnormal increase in the intraocular pressure and a lesion in the optic nerve with anatomic alterations of the ocular globe. It is not associated with some other pathological conditions. This kind of glaucoma is a very important cause of blindness in childhood. There is variability in the clinical presentation depending upon the degree of abnormality in the angle at the cornea. There is a classic triad composed of increased lachrymal production, photophobia and blepharospasm. The diagnosis is based upon the quantification of the intraocular pressure. Measure of the corneal diameters, gonioscopy, anterior Segment exploration, retinoscopy, echography is also done. The treatment of the congenital glaucoma is fundamentally surgical, but the medical management plays an important role before the surgery takes place. The goniotomy is the treatment of choice. Trabeculotomyis the technique of choice in children under three years of age with enough corneal opacity. Cyclodestructive proceduresare indicated in the pediatric glaucoma in cases of absolute glaucoma with elevation in the intraocular pressure and pain. The group of medications commonly use are pilocarpine, Beta blockers, Carbonic anhydrase inhibitors and Prostaglandin analogs. The infantile glaucoma is a relatively low frequency disease that implies a bad visual prognosis.
