Abstract

Pulmonary embolism (PE) is caused by the occlusion of the pulmonary arterial bed by clots from the veins of the systemic circulation that embolize in the lungs. It is most often a consequence of deep venous thrombosis (DVT). The hemodynamic response to PE depends on the size of the embolus, concurrent cardiopulmonary disease and neurohumoral effects. The clinical presentations of acute PE are nonspecific, ranging from shock or sustained hypotension to mild dyspnea. Rapid risk stratification facilitates selection of patients who warrant aggressive intervention with thrombolysis or embolectomy. For patients with idiopathic venous thromboembolism, there seems to be a lifelong tendency for recurrent thrombosis unless anticoagulation is continued.
