Cancer is the second most common cause of death in the developed countries. Other than routine chemotherapy, neurologic problems are the most common reason for the hospitalization of cancer patients. Neurological manifestations of systemic malignancy may be due to metastases, or infiltration neuropathies or indirect effects such as metabolicencephalopathy, paraneoplastic syndromes. Neurological manifestations may be metastatic and nonmetastatic. Brain metastases are the most common; most of whom have multiple metastases. Whole-brain radiation therapy remains the standard of care. Surgery for single accessible lesion and SRS for in accessible brain metastases appear to be effective. Aggressive focal therapies may do well, achieving a median survival of about 1 year, with some patients surviving several years. Paraneoplastic syndrome is the most common nonmetastatic complication. Treatment is not satisfactory.
