Abstract

Lamellar keratoplasty is broadly categorized as either anterior or posterior keratoplasty. Anterior lamellar keratoplasty is indicated in eyes where the pathology spares the endothelium; endothelial keratoplasty is indicated where the pathology is confined to the endothelium. Deep anterior lamellar keratoplasty is the preferred surgical treatment option for corneal opacification or disease in which the endothelium is healthy. It is preferred over penetrating keratoplasty primarily because it minimizes the risks of intraocular complications and immune graft rejection. It is, therefore, ideally suited in ‘high risk’ keratoplasties like in young patients and in vascularized corneas where the corneal pathology spares the endothelium or where large grafts are indicated.
