Abstract

The main objective of any dermatosurgical procedure is to achieve a good cosmetic outcome. Infection at the site of surgery is one of the important factors that determine the outcome of surgery. All dermatosurgical procedures result in damage to the skin barrier to a lesser or greater extent, thereby predisposing the patient to the risk of infection. Though prophylactic administration of antibacterial agents minimizes the risk of infection, lack of data on their role in dermatosurgery is leading to inappropriate use and development of bacterial resistance. Furthermore, antibacterials can cause adverse reactions and rarely serious anaphylactic reactions. There are also known interactions between and antibacterials and several medications administered for underlying health problems. Therefore, there is a need to understand the factors responsible for infection and the role of antibacterial agents in dermatosurgery, and to follow appropriate guidelines for the use of anitibiotics, both for prophylaxis and treatment of infections. The purpose of this chapter is to review the literature regarding the use of topical and systemic antibiotics prophylactically, intraoperatively and postoperatively in various dermatosurgical procedures. Although the primary focus will be on traditional excisional or cold steel surgery, the use of these medications for more specialized procedures in the field of dermatology such as facial resurfacing, chemical peeling, liposuction and other cosmetic surgical procedures, will also be briefly addressed.
