Abstract

This chapter discusses about donor kidney scoring systems used to predict graft function, where this systems have implications for organ utilization and potential to improve graft survival with proactive modification of immunosuppression. Biopsy evaluation of donor kidney is particularly important for marginal organs to determine organ suitability. Maryland aggregate pathology index, MAPI is an aggregate pathology score that is based on the evaluation of donor pathological features that are predominantly related to ischemic and hypertensive injury affecting glomeruli and vessels. Kidney donor risk index, KDRI, is a continuous donor scoring system which was developed by analyzing 69,440 donor recipient pair data from scientific renal transplant registry in order to measure the risk of the various factors known to influence graft failure. Remuzzi score is a subjective semi-quantitative scoring system for evaluation of donor kidneys above 60 years of age, and estimate number of nephrons and the suitability of the organ as single or dual transplant.
