Abstract

The first 6 months post-transplant is usually considered the period of the highest risk of rejection and is the point of greatest immunosuppression and the poorest response to vaccination. Transplant patients should receive all age-appropriate and risk-specific vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) Centers for Disease Control and Prevention prior to transplantation. Inactivated vaccines are safe in the post-transplant patient receiving immunosuppressive therapy. There are several small studies in the pediatric population that showed live attenuated vaccines, such as MMR and varicella may be given safely to seronegative post-transplant patients. Large studies will be needed to identify the post-transplant population who failed to receive pretransplant vaccines and whether post-transplant patients who remain seronegative may benefit from these live vaccines.
