Abstract

This chapter discusses principles of rhinoplasty, where human nose is a complex three-dimensional structure comprised of the external skin and soft tissue envelope, an osseocartilaginous framework, and an internal mucosal lining. The nasal skin is generally thin along the upper half of the nose and thickens as it approaches the nasal tip. The nasal musculature lies within the fibromuscular layer of the nose within the superficial musculoaponeurotic system. The cartilaginous structures of the nasal framework lie caudal to the bony structures. The upper vault is the bony vault comprised of the paired nasal bones and the paired ascending processes of the maxilla. The middle vault is a cartilaginous vault and is composed of the upper lateral cartilages and the cartilaginous dorsal septum. The lower vault defines the nasal tip that is shaped by the paired lower lateral cartilages. Anderson’s tripod model is a simplified method of understanding how the crural length and shape of the lower lateral cartilages affect the nasal tip. Photography is an essential tool in rhinoplasty.
