Abstract

Aortic valve disease is one of the most common cardiac ailments. Rheumatic heart disease (RHD), bicuspid aortic valve (BAV) disease and senile degeneration account for more than 90% of all aortic valve lesions. The definitive treatment of aortic valve disease is the aortic valve replacement (AVR), at least in adult patients. This chapter discusses some of these important challenging issues encountered in regular clinical practice. Aortic stenosis can be treated by valve replacement by an artificial valve. Aortic stenosis in association with other valvular diseases can be managed by identifying the appropriate approach. AS in pregnancy is associated with hemodynamic changes that start to appear during the first trimester itself. Aortic valve surgery is an alternative but is associated with fetal mortality as high as 20%. Certain challenges are associated with Aortic Stenosis in Association with Asymmetrical Septal Hypertrophy (ASH). In addition, aortic regurgitation, diagnostic issues in assessing the severity of AR, and bicuspid aortic valve in absence of severe AS or AR are discussed.
