Abstract

The chapter comprises on gaps and challenges in antiplatelet therapy with Indian perspective. Antiplatelet drugs remain the cornerstone in the treatment of CVD. Underlying mechanism of acute coronary syndromes (ACSs) is most commonly due to rupture or erosion of an atherosclerotic plaque resulting in a thrombotic event. There are several categories of antiplatelet agents, acting through various pathways that inhibit platelet activation. The chapter mentions important points on antiplatelet agents such as aspirin, adenosine diphosphate receptor antagonists which includes clopidogrel, prasugrel, ticagrelor and intravenous antiplatelet drugs cangrelor. It also highlights on antiplatelet drugs under development, guidelines for antiplatelet therapy, current choice of antiplatelet therapy: Indian perspective. Finally the chapter concludes with remarks as The recent guidelines supports the use of any of these newer antiplatelet agents with the final decision based on patient presentation, local institutional protocols, risk factors, management strategies and not the least the cost-effectiveness of these drugs.
