Abstract

The present chapter comprises on the antiplatelet drug-resistance in Asian population. It has been considered that aspirin and clopidogrel have become standard therapy in patients after myocardial infarction and percutaneous coronary intervention (PCI) with stenting. The lack of standard definition of resistance as well as diagnosing modality has hampered the identification and the treatment of this clinical entity. Aspirin acts by irreversibly acetylating the COX-1 enzyme, thus suppressing thromboxane A-2.1 Thromboxane A-2 serves as a potent agonist of platelet aggregation, and aspirin prevents thrombus formation via this mechanism. Clopidogrel is a prodrug that is converted in the liver by cytochrome P450 3A4 (CYP3A4) into its active metabolite. It inhibits platelet aggregation by irreversibly binding to the P2Y12 ADP receptor on the platelet surface. The chapter also marks important points on genetic variability on platelet function test, treating aspirin and clopidogrel resistance and future direction and clinical consideration.
