Abstract

Rotational atherectomy (RA) is a technique used for the endovascular treatment of obstructive atherosclerotic coronary artery disease. It is a complementary procedure which facilitates angioplasty and deployment of stents in complex lesions particularly those affected by heavy calcification. In this chapter mechanism, equipment, procedure, lesion selection, indications and outcomes of RA are discussed in detail. RA should be used as a tool to make PCIs possible in complex lesions with moderate or severe calcification when clinical variables make PCI appropriate. RA remains an integral tool to permit optimal angiographic outcomes in treatment of complex coronary artery disease involving moderately to severely calcified lesions.
