Abstract

The syndrome of resistance to thyroid hormone is characterized by reduced clinical and biochemical manifestations of thyroid hormone action. Clinical features for the diagnosis of RTH includes Palpitations and Resting Tachycardia, Neurologic and Neuropsychologic abnormalities, high serum free T4 and a nonsuppressed TSH etc. Other aspects of the disease like differential diagnosis and molecular genetics also described completely. Eighty percent of RTH is familial, dominantly inherited and associated with homozygous mutation in the TR-β gene. The management of RTH attempt to treat the biochemical abnormalities with surgery or radioiodine, by b-blockers, agents like TRIAC to inhibit TSH secretion etc., are used to overcome clinical challenges that arise from RTH.
