Abstract

This chapter discusses laparoscopic suturing of the bowel and bladder injury. During pelvic surgery, a gynecologist will most commonly encounter the intestinal tract in two clinical situations: dealing with endometriosis and dealing with adhesions. For a laparoscopic surgeon, a third possibility of encountering the intestinal tract is during trocar placement. The repair of bowel wounds laparoscopically is dependent largely on two factors—preoperative bowel preparation and surgeon’s suturing capabilities. The incidence of bladder injury during laparoscopic procedures ranges from 0.02% to 8.3% of cases. Under magnification provided by video laparoscope, a skilled laparoscopist should be able to repair even a large intentional or unintentional cystotomy laparoscopically. The chapter covers intraoperative diagnosis of bladder injuries, technique of bladder repair, associated pitfalls, and postoperative management of bladder injury.
